消毒用マウスウォッシュによる口腔ケアの徹底
患者衛生の一環として、口腔ケアは 口腔内の細菌量を一時的に減少させるための重要な要素である。細菌は、特に危険因子を持つ患者において、歯周病（歯肉炎、歯周炎）や感染症を引き起こす可能性があります1 消毒性の洗口液は、細菌およびレボリューシス性細菌の負荷を軽減するのに役立ちます2。
消毒用マウスウォッシュでリスクを防ぐ
最初の細菌は、歯磨きからわずか2時間後に再び定着し始めます3。時間が経つにつれて、他の、ほとんどが無害な細菌が加わり、糖分とタンパク質のマトリックスを形成し、バイオフィルム（歯垢）が形成されます。その約2日後には、病原性を持つ可能性のある細菌が定着します4。入念なデンタルケアは、細菌の定着を抑制するのに役立ちます。口腔衛生に問題がある場合は、機械的な歯面清掃に代えて、殺菌効果のある洗口剤を使用することもできる。
包括的な口腔ケア（歯磨きと歯科衛生）を行わないと、72時間以内に硬くなった細菌の沈着物からなる歯垢が形成される4。 その結果、口腔内細菌叢がもともとレンサ球菌や放線菌であったものから好気性のグラム陰性菌へと変化する5。その結果、歯周病や感染症などの健康被害が発生し、予定していた手術を延期しなければならなくなることもある。6

呼吸器感染症の予防
歯磨きやデンタルケアによる最適な口腔衛生は、介護施設や病棟では必ずしも実現可能ではない。そのため、歯周病や口腔鵞口瘡、重篤な肺炎を引き起こす可能性がある。ドイツでは、人工呼吸器関連肺炎（VAP）は最も一般的な病院感染症のひとつである13。 VAPの場合、在院日数は6～9日延長され14,15、患者1人あたり最大30,000ユーロの追加費用が発生する可能性がある16,17,18,19,20 。16,17,18,19,20
院内人工呼吸器関連肺炎を予防するために、KRINKOガイドライン「院内人工呼吸器関連肺炎の予防」では、特に有効性が証明されている消毒物質による定期的な口腔ケアを推奨している21。
オクテニデント® 消毒性洗口液
オクテニデント® 消毒性洗口液の使用は、口腔衛生スキルの不十分な成人の歯磨きの一時的な代替となり、以下のことに貢献する：
- 口腔内の細菌数の一時的減少。
- 歯垢形成の一時的抑制。
この承認薬は、有効成分オクテニジン二塩酸塩（オクテニジン）を含有しています。オクテニジンは殺菌作用（グラム陽性菌およびグラム陰性菌）および 殺レブロシド 作用 *を 有し、口腔鵞口瘡の主な原因菌の1つである カンジダ・アルビカンスに対しても試験管内で有効です。また、この消毒用洗口液は忍容性が高く、全身への吸収が低いため、妊娠中**にも使用できます。

使用方法
患者：1日2回、できれば食後にオクテニデント®消毒薬を使用してください。歯磨き後すぐに使用する場合は、オクテニデント®消毒薬を使用する前に、口腔内を水で十分にすすいでください。付属の計量カップに10mlの消毒用洗口液を入れ、約30秒間口を十分にすすいでください。その後、溶液を吐き出す。これは飲み込まないこと。直後に水で口をすすがないこと。この薬は5日以上連続して使用しないでください。
octenident® antiseptic 1 mg/ml oromucosal solution - Active substance: octenidine dihydrochloride. Composition: 1 ml of solution contains 1 mg of octenidine dihydrochloride. Other ingredients: glycerol 85 per cent (E 422), sodium gluconate, citric acid, disodium phosphate dihydrate (for pH adjustment), macrogolglycerol hydroxystearate, sucralose, purified water, mint flavour (contains propylene glycol (E 1520)). Indications: octenident antiseptic has an antimicrobial effect. It is used for temporary reduction of bacterial count in the oral cavity, for temporary inhibition of plaque formation, and in cases of insufficient oral hygiene capacity (no toothbrushing possible, for example) in adults. Contraindications: Allergy to octenidine dihydrochloride or any of the other ingredients. Undesirable effects: Very common: Temporary taste disturbance, such as bitter aftertaste; Mild, reversible dental discoloration. Common: Numb sensation in the mouth, Coating of the mouth or the tongue, Temporary tongue discoloration, Sensitivity of teeth. Uncommon: Headache, Nausea, Tingling of the tongue, More saliva in the mouth than normal.Revision 09/21
